Passport Application Procedure, DFA Consular Office at Robinson's Place, Lipa City, Batangas

The DFA Regional Consular Office in Lipa City, Province of Batangas, is pleased to announce that it will officially launch its online appointment system beginning 22 July 2016. The move is in line with the Department’s E-passport system upgrade, to further improve its frontline service by providing the public the convenience of obtaining online appointment date and time for their passport applications.

On 29 July 2016, application forms will no longer be distributed and DFA Lipa will only accept applicants with confirmed online appointments.

Exempted from obtaining confirmed online appointments are the following:
  • Senior citizens
  • Minors aged three (3) years old and below
  • OFWs with contracts/job orders
  • PWDs
  • Pregnant women
The exemptions are, however, subject to certain conditions.

DFA Lipa Application Procedure

The procedure below is condensed from official procedure posted outside the DFA Regional Consular Office at Robinson's Place, Lipa City.  This is the procedure that is to be followed by those who have obtained online appointments at the consular office.

Requirements

The information contained below has been taken from documents posted outside the Consular Office of the DFA at Robinson's Place, Lipa City, Batangas.

Requirements for First-Time Passport Applicants (18 And Above)

General Requirements:
  • Personal appearance
  • Birth certificate in security paper issued by the NSO.
  • Marriage contract in security paper issued by the NSO (for married women only).
  • Certified True Copy/Transcribed Copy of birth/marriage certificate when entries in NSO BC/MC are blurred or not readable.
List of acceptable ID cards:
  • Digitized SSS ID
  • Driver's License
  • GSIS ID
  • Digitized BIR ID
  • Voter's ID
  • OWWA ID
  • IBP ID
  • PRC ID
  • Senior Citizen's ID
Other acceptable ID cards:
  • Old college ID
  • Alumni ID
  • Old employment ID
List of supporting documents (at least three of the following) old public document issued at least one year prior to the date of the application that show correct name, date of birth and place of birth, picture and signature of applicant:
  • NSO marriage contract
  • Land title
  • Elementary or High School Form 137 with readable dry seal
  • Government service record
  • NBI clearance
  • Police clearance
  • Barangay Clearance
  • NSO Birth Certificate of Child/Children
  • SSS E-1 form
  • Income Tax Return
  • Voter's certification, List of voters, Voters' Registration Record (please attach receipt)
  • Yearbook
  • Community Tax Certificate
If birth was late registered, submit documents with correct date and place of birth:
  • Baptismal certificate
  • Form 137 with readable dry seal
  • Voter's certification, List of voters, Voters' Registration Record (please attach receipt)
For women married to foreign nationals:
  • Original and Photocopy of Commission of Filipino Overseas (CFO) Guidance and Counseling Certificate
NOTE: All documents are subject to checking and approval of the processor. The Department may require additional documents as may be necessary.

Requirements for First-Time Passport Applicants (Below 18)

General Requirements:
  • Personal appearance
  • Birth certificate in security paper issued by the NSO
  • Marriage contract in security paper issued by the NSO (Parents)
  • Certified True CopyTranscribed Copy of Birth/Marriage certificated when entries in NSO BC/MC are blurred and not readable
  • Personal appearance of either parent
  • ID (School ID/Form 137) of minor 8-17 years old for first time and renewal applicant.
  • For minor who never attended school, a notarized affidavit of explanation executed by either parent detailing the reasons why the child is not in school is required
  • Original and photocopy of valid Passport of travelling companion
Not Travelling with Either Parent or if Travelling Alone:
  • Original and photocopy of DWSD travel clearance
  • Affidavit of support and consent indicating the name of travelling companion. If minor is illegitimate, mother should execute the ASC.
If Both Parents are Abroad:
  • Affidavit of support and consent indicating the name of travelling companion.  With attached copy of valid passport of parents  and MUST BE AUTHENTICATED BY THE NEAREST PHILIPPINE EMBASSY OR CONSULATE.  Commonly referred to as "red ribbon."
  • Special Power of Attorney designating the representative by name and authorizing him to apply for the minor's passport and must be AUTHENTICATED BY THE NEAREST PHILIPPINE EMBASSY OR CONSULATE.
  • Copy of valid passport(s) of either or both parent(s).
  • Original ID of the duly authorized representative.
If Minor is Illegitimate and Acknowledged by Father:
  • Amended NSO Birth Certificate reflecting surname of father with acknowledgment and consent to use the surname of the father.
If the Minor is Legitimated by Subsequent Marriage:
  • Annotated Birth Certificate from the NSO regarding the new status as legitimated and the full name of the child correctly stated.
  • NSO Marriage Contract
NOTE: All documents are subject to checking and approval of the processor. The Department may require additional documents as may be necessary.

Requirements for Renewal:
  • Personal appearance
  • Original passport to be renewed
  • NSO Birth Certificate if there is correction on the details listed in the  data page of the passport to be renewed
  • Affidavit of Discrepancy/explanation for correction
Renewal of Brown Passport:
  • Old passport and photocopies of passport pages 1, 2, 3, 4 and last.
  • Birth certificate from NSO to determine the complete middle name.
Renewal of Green Passport:
  • Original green passport and photocopies of first and last pages.
Renewal of Machine Readable Passport Issued 2007:
  • Original passport and photocopies of first and last pages.
  • Birth certificate from NSO to determine the complete place of birth or old cancelled may be presented in the absence of the NSO Birth Certificate.
Renewal Change Status:
  • Marriage contract issued by the NSO.
  • Marriage contract from the Local Civil Registrar when entries in the NSO copy is blurred and not readable.
For Woman Married to a Foreign National:
  • Counseling Certificate issued by the CFO Renewal Married to Single Name
  • NSO Birth Certificate
  • NSO Marriage Contract with annotation of NULL AND VOID MARRIAGE
  • NSO Death Certificate of Husband (for widows who wish to revert to their single names).
NOTE: All documents are subject to checking and approval of the processor. The Department may require additional documents as may be necessary.

Requirements for Replacement of Lost Passports:

If Passport is Expired or Not Valid:
  • Personal appearnce
  • Affidavit of loss
  • Submit same documents required from the first time applicant
If Lost Passport is Still Valid:
  • Personal appearance
  • Affidavit of loss
  • Submit same documents required for first time application
  • Police report
  • 15-day clearing period prior to the processing of application for replacement of lost valid passport
Lost valid Passport Fee:
Additional PHP 200.00 (for lost MRRP/MRP)
Additional PHP 350.00 (for lost ePassport)

NOTE: All documents are subject to checking and approval of the processor. The Department may require additional documents as may be necessary.

Requirements for Passport Applicants who have Reacquired Philippine Citizenship Under R.A. 9225/With Dual Citizenship:
  • Birth Certificate/Report of Birth authenticated by the NSO
  • Old Philippine Passport (if still available)
  • Original and photocopy of foreign passport 
  • Oath of Allegiance from the Bureau of Immigration or FSPs
  • Identification Certificate
  • Order of Approval
NOTE: All documents are subject to checking and approval of the processor. The Department may require additional documents as may be necessary.

Announcement post outside the DFA Consular Office at Robinson's Place, Lipa City to holders of Green or MRP Booklets:

Effective 6 June 2016, all applications for Passport Renewal for holders of Green or MRP booklets shall only be processed upon submission by the applicant of the following requirements:
  • Actual MRP/Green Passport
  • PSA-Authentic Birth Certificate and Marriage Contract if Married
  • At least one (1) valid ID (SSS, Voter's or Driver's License)
